The Good Samaritan Health Clinic serves the uninsured people of Cullman County, AL without access to healthcare. We see patients in our Medical Clinic Monday, Tuesday, Thursday and Friday 8am to 12noon and 1pm to 4pm, and Wednesday 8am to 12noon. Dental Clinic sees patients every 2nd and 4th Monday of each month.

Details: Our clinic provides primary care to residents of Lee County, Alabama who do not have medical insurance, Medicare, Medicaid (ALLKids), or VA Benefits. To become a patient, you must go through a screening process. We provide many primary care services in the clinic. We also work to find affordable medications by utilizing Patient Assistance Programs from drug companies and other groups that make medicine more affordable. We are also mindful of the discount drug lists at local pharmacies. Most visits also include the opportunity to meet with a spiritual counselor.
Remarks: We are usually open for patient screenings and medicine pick-up 9-11am Monday-Thursday. Patients can also pickup medicine 1-3pm Monday-Thursday and during after-hours clinics. Call before coming to the screening to ensure we are screening that day.

These community-minded individuals are the heart and soul of our clinic. Who the Clinic Serves: Adults - 18 or older; Baldwin County resident; No private medical insurance, Medicaid, or Medicare Parts A or B; Must provide proof of income; Preference is given to individuals with high blood pressure, diabetes, elevated cholesterol, pulmonary disease and related conditions.

Community Of Hope Health Clinic (chhc)

Details: The Community of Hope Health Clinic (CHHC) is a medical mission in Shelby County that provides non-emergent medical care to Shelby County adults who have no health insurance and whose household income is at or below 200% of the poverty level. The Clinic opened its doors on October 16, 2008 after entering into a Memorandum of Understanding with the Shelby County Health Department for use of their building on Thursday evenings. We have two part-time paid positions, the Executive Director and an Administrative Assistant. Other than these two positions the CHHC is totally manned by volunteers. We utilize volunteers from the following disciplines: physicians, nurse practitioners, nurses, social workers, chaplains, and interpreters. We continue to receive a huge in-kind value provided by our many volunteers. We also have physicians in the area who provide pro bono medical care for referrals that we make. The eligibility criteria are: You must live in Shelby County and provide verification of this (ex: utility bill); You must not have ANY health insurance at all; You must be between the ages of 19 and 64; You must be at or below 200% of the poverty level based on total household gross monthly income (HH 1 can have $1805; HH of 2 can have $2,428; HH of 3, $3,052; HH of 4, $3,675; HH of 5, $4,298). Income verification can be a paystub, an award letter for Disability or Unemployment Compensation, a statement from the person who might be currently supporting you. If you are on Food Stamps, you automatically meet our financial guidelines.
Remarks: Hours of Operation: Thursday-5 to 8:30 PM; Monday-9 AM to 3 PM. An appointment is necessary for both of these Clinics. To obtain an appointment you must go to the Community Room of the Shelby County Health Department on a Thursday morning at 10:30 AM
