Today we serve all residents of the Santa Clarita Valley and beyond. Our mission is to enhance wellness by providing quality outpatient health care services and programs, health education, community resources, and referrals. Our philosophy is to empower patients to manage their health and health care. We follow the self-management patient care model in which patients take a central role in managing their health.
