The Alamosa Family Medical Center (AFMC) is part of Valley Wide Health Systems, Inc. which was established in 1976. This clinic and the Valley Wide Pharmacy are centrally located in Alamosa, directly across the street from the Adams State University Nielsen Library, and serve all surrounding communities of Rio Grande County, Conejos County, Costilla County and Saguache County. AFMC offers family practice care for all members of your family no matter what age. The clinic specializes in Obstetrics/Gynecology (OB/Gyn) to care for women before and after childbirth, continuing with pediatric care for your newborn through all developmental stages. Through the use of special programs like Bright by Three and Reach Out and Read promotion for early childhood literacy and a love of books and reading becomes a standard part of early childhood primary care and has been offered in Valley-Wide clinics since 1998. AFMC offers ancillary services for breast feeding education, and provides a private lactation support room that is available to the general public. Family Planning services are also available to help you decide what is “best” for your family planning goals. We offer easy access to FREE pregnancy testing and confidential adolescent visits. FREE HIV screening services and a full spectrum of STD testing and treatment are available in a safe and confidential setting. We take a team based approach to your care. Your primary care provider, nurse team and support staff will ensure a pleasant visit and will provide care for your acute, chronic, preventive, reproductive and behavioral health needs. We can also offer immigration medical exams. After your visit, our case managers can help you select a Primary Care Provider and/or help you look into some insurance or health coverage options if you do not currently have insurance. They can also evaluate you for Medicaid eligibility. All Valley-Wide Health Systems sites are now on an Electronic Health Records system. This allows for immediate access to your health records in any other Valley-Wide medical, dental, physical therapy or anti-coagulation clinic. Valley-Wide Health Systems will also send your prescription directly to your chosen pharmacy electronically when allowable.

The mobile clinic consists of a full medical exam room and a full dental operatory, although each room can be utilized for either function. Heart and Eagle creates a mobile single point of entry to address medical, dental and social service needs, outside of the traditional clinic setting.     • Details: In 2008 Valley-Wide Health Systems, Inc. developed the convenient care model and established Convenient Care Community Clinic to give patients and the community an alternative to the local emergency room for acute and non-urgent care needs outside of traditional clinic hours. Convenient Care Community Clinic (CCCC) originated as part of the Sierra Blanca Medical Center, but was later relocated to a separate facility at 1131 Main Street in Alamosa. The CCCC model not only offers patients a convenient option for illnesses outside of regular office hours, for outpatient treatment, minor injuries, and/or acute illness, but also provides better access to walk-in, same day services which are available 7 days a week including holidays. CCCC serves as an alternative to the cost of an emergency room visit for problems on the lower end of the severity spectrum. Visits should be for acute care only and should not replace visits to your Primary Care Provider or regular physician. Refills for chronic medication prescriptions cannot be requested through this clinic. CCCC also offers FREE HIV screening services and a full spectrum of STD testing and treatment in a safe and confidential setting. CCCC does not require that you be a Valley-Wide Health Systems patient, and is open to both the community and to visitors needing services while in the San Luis Valley. After your visit, treatment or screening, our Case Managers can help coordinate follow-up care with your Primary Care Doctor or help you select a Primary Care Provider. CCCC is located within walking distance to both the Adams State University and Trinidad State Junior College Campuses and Walgreens pharmacy. The behavioral health consultant works with a primary care provider to evaluate the mind-body-behavior connection and provide brief, solution-focused interventions. ASHC offers family medicine care for all, no matter what age. The primary care provider, nurse team and support staff will ensure a pleasant visit and will provide care for your acute, chronic, preventive and reproductive health needs. Care Coordination, Case Management and Financial Eligibility staff will be available to complete comprehensive needs assessments to identify and overcome barriers to care. Financial assessments for Medicaid, CHP+, Colorado Indigent Care Program (CICP) sliding fee discount program (SFDP) and other health insurance, as well as, health wellness programs will be offered at ASHC.
