Clinic Details:Primary Health Care (PHC) has been providing medical and dental care to underserved individuals and families living in Des Moines since 1981. Today, we provide care and services in Ames, Marshalltown and Des Moines. PHC has three dental clinics, seven medical clinics and four satellite clinics across these communities. PHC also operates a mobile health unit, which provides care where needed in our service area. Based on the needs of our communities, PHC offers a spectrum of medical and dental services including family practice, internal medicine, pediatrics, OB, Title X services, substance abuse, behavioral health, oral medicine, HIV care and services, pharmacy, lab and x-ray. Enabling services are available to help patients with entitlements, case management, transportation, translation and patient education. PHC employs 52 full time physicians, dentists, physician assistants, nurse practitioners, dental hygienists and licensed social workers. PHC has 375 employees. In 2016, our clinics and programs served 36,898 patients and clients with over 145,988 encounters.

Details: Jim Ellefson ran a pharmacy for many years at 1607 E. 33rd St. in Des Moines. Following his death in 2001, his wife, Judy, donated the building to the Free Medical Clinic which had been operating out of Evangelical Covenant Church at 2625 Hubbell Ave. Jim and Judy Ellefson had helped support the clinic since its inception in January 1997 by donating prescription drugs. Dr. Nick Palmer was the founder of the clinic and one of the many doctors volunteering at the clinic. He had been forced to give up his family practice due to declining health from Crohn’s disease. But Dr. Palmer saw a new opportunity through the free clinic to fulfill his dream of caring for others. The donated 80-year-old, 1400 square foot building was renovated by volunteers and opened for business in October 2001. The opening of our new “free-standing” clinic made the operation and access to the clinic much easier for everyone. We no longer had to pull everything out and set up every time. It allowed us to expand the medications dispensed to patients. We also added a “surgery room” that allowed the doctors to perform small office surgeries. One of the possible developments from Crohn’s disease is cancer, which took the life of Dr. Palmer in September 2009. His passing was a great loss to the east side and to those with no other options for medical care. Dr. Palmer’s wife, Val, has continued to operate the clinic to carry on Dr. Palmer’s vision. All of our volunteers are unpaid, and many have volunteered since we began…nothing feels as good as giving!

Details: The PHC Pharmacy provides medication to patients seen at our clinics. As a 340(B) pharmacy, we are able to provide medications at a reduced cost to eligible patients. Patients may also qualify for other assistance programs. Clinical pharmacy services are also available to help patients with chronic health conditions. Consultations with the pharmacist about medications, glucometers, diabetes management and nutrition are also available.
Details: The goal of The Project of PHC is to provide confidential, free or low cost services to help people living with HIV move through the stages of HIV medical care. From the time of diagnosis, our staff help patients connect to care, access resources that will help them achieve the best possible health outcomes and overcome obstacles that many stand in their way to a long and happy life. We also focus on prevention, providing outreach and access to the most effective tools to prevent HIV infection.
Details: Proteus provides health care services to migrant and farmworkers located in Iowa. Patients are seen at mobile clinics, in-house clinics and on an individual basis by a Proteus contracted licensed medical professional. Proteus uses a sliding fee schedule based on income for health services. Vouchers are provided for other health services with the many partnership agreements with other medical professionals in the state. These include specialty providers, dentists and vision experts. Proteus bilingual community health aides provide transportation and interpretation for these other services when needed.
