  • Clinic Details: Located in Falcon Elementary School, our Falcon Peak Health Center provides outpatient care for your children, from 0-21 years of age, who attend or has a sibling enrolled in District 49. We ensure that your child’s well-being is safeguarded as he or she grows from childhood into adolescence. In addition to caring for illnesses and injuries and chronic health issues, we provide preventive physical exams, immunizations and Behavioral Health Providers to help with emotional or behavioral concerns. We also provide you with knowledgeable Outreach/Enrollment Specialists who will work together with you to provide screening and processing of applications for Medicaid, CHP+ and CICP. Many children in District 49 qualify for one of these programs.
