Clinic Details:MayView Community Health Center (MayView) is a nonprofit primary care clinic devoted to providing healthcare to low-income families and individuals throughout northern Santa Clara County with clinics located in Palo Alto, Mt. View and Sunnyvale. MayView staff celebrate and share the rich cultural and language backgrounds of our patients and community. We practice a culture of health and speak the language of caring. Details: The Sunnyvale outpatient clinic is one of 11 operated county-wide by Santa Clara Valley Medical Center (SCVMC), and opened its doors in 2008. Here, you’ll find the same high-quality care and coordination available at all SCVMC locations. Our friendly and highly-qualified doctors, nurses, and staff offer a wide variety of services, including adult and family medicine, Ob/Gyn, pediatrics, surgical services, lab, radiology/diagnostic imaging, convenient on-site pharmacy, and more.

Details: For nearly five decades, Gardner Health Services has provided exceptional health care to people of all ages, backgrounds, and life circumstances. In 1967, community members with help from the Stanford Medical School created a small clinic to meet the urgent health needs of local farm workers in the Alviso community. Since that time Gardner has expanded in response to community needs, now providing medical, dental, vision, counseling, and substance abuse services to more than 55,000 Santa Clara County residents annually. At Gardner, we know that having a place to go for regular medical care is crucial to keeping people healthy and reducing the cost of healthcare for taxpayers. Your support ensures Gardner can continue to provide comprehensive, high quality medical and mental health care to our most vulnerable community members.
Details: Gardner Family Health Network’s Health Care for the Homeless (HCH) program continues to maintain its excellent relationships with community resource agencies in Santa Clara County and develops new relationships as community services develop and the needs of homeless people change. We have agreements to deliver health services during hours that homeless people are present at key homeless shelters and service sites from Palo Alto to Gilroy, including EHC/LifeBuilders, InnVision, St Joseph’s, Mountain View Day Worker Center, Bill Wilson Center, Georgia Travis Center, Salvation Army and other locations. Because of the high concentration of the homeless and homeless service sites in downtown San Jose, the new Gardner Downtown Health Center is expected to increasing the number of homeless we serve from 3,000 to 4,000 individuals and increase the number of clinic visits from 10,000 to 13,000 for individuals and family members who happen to be homeless.
