Clinic Details:Teen and young adult health is the focus of the Adolescent Medicine. Our team looks at each teen and young adult as an individual and considers every aspect of their lives in order to address their unique medical and mental health issues. Board certified physicians, psychologists, psychiatrists, social workers, nurses, and specialists in health education and substance abuse provide primary and specialty care for youth, teenagers, and young adults ages 11-24. UCSF Benioff Children's Hospital Oakland is a leader in providing adolescent medicine community-wide. We have three community health centers dedicated to providing comprehensive care, each of which serves as a national model for adolescent healthcare.

Details: Mobile Health provides urgent care, brief social work assessments, and referrals to community resources while visiting shelters, meal programs, and other accessible locations in the community. Staff from Alameda Health System and Health Care for the Homeless include a medical provider, medical assistant, social worker, registration clerk, and van driver who work together to provide friendly and accessible care to those who need it most, as well as linkages to ongoing primary care at Alameda Health System or other community clinics.

Details: LifeLong TRUST Health Center recently opened, serving adults with disabling conditions and histories of homelessness who also participate in Alameda County’s General Assistance Program. This collaboration between multiple government agencies and non-profit organizations will promote dignity, respect, and compassion in all interactions among staff and clients.
