Clinic Details:Homeless individuals are up to six times more likely to get sick than the general population. Compounded by lack of access to quality healthcare, chronic illness creates instability that makes it difficult to secure housing and employment. The shelter-based clinics located in our Third Street, Fort Washington, New Providence, and Williams Avenue shelters serve homeless adults from all over New York City. Our physicians provide compassionate care and no one is turned away, regardless of their ability to pay. Services include diabetes testing, depression and substance abuse screening, HIV testing and viral load suppression, tobacco cessation, and detox treatments - all crucial for the population we serve. Addressing these health issues give homeless individuals a chance to get back on their feet. Additionally, since homeless individuals visit the emergency room an average of five times a year at a cost of $3,700 per visit, our clinics allow more efficient, effective use of healthcare resources by reducing non-urgent ER use and preventable hospitalizations. Our primary care program is accredited by the National Committee on Quality Assurance (NCQA) as a Patient-Centered Medical Home.
Details: The Community Medicine Program - Family Health Centers at NYU Langone provides healthcare to adults through several New York City homeless services locations. Our program reaches people who otherwise would have limited access to healthcare. We provide primary and urgent care as well as vaccinations, women’s health services, dental care, and health education. We also offer behavioral health services including psychotherapy and psychiatric evaluation. In addition to our primary care services, we can connect our patients with doctors from more than 25 medical and surgical specialties. Treatment for substance use is available to residents of the shelter system and those who are formerly homeless and living in the city’s supportive housing. Services include group therapy, supportive therapy, and medication. We also provide referrals and navigation services to help people enter detox programs and addiction rehabilitation. Our care team includes doctors, psychiatrists, nurse practitioners, registered nurses, psychiatric nurses, social workers, HIV counselors, care managers, and substance abuse counselors.

Our program makes health care services easily accessible to people experiencing homelessness. We provide medical care, behavioral health care, discounted medications, and referrals to other services. The program is funded by Care for the Homeless, which receives federal and other funding. Care for the Homeless staff provide HIV intensive case management, substance abuse counseling, HIV rapid testing and HIV and general health education at many of the sites.
