Clinic Details:Homeless patients and families can access all services at SWCHC, with additional interventions available to meet the specialized needs of this population. A team of clinicians and support staff visit 10 licensed shelter clinics on a rotating weekly schedule, with referrals for specialty care, and intensive medical follow-up performed at the sites as needed. In addition, the Homeless Team offers health care services to “street” homeless at non-traditional locations such as parks, bridges and feeding vans.
Details: Our Clinton Avenue site specializes in women’s health. Complete gynecological care includes prenatal care, pregnancy testing, management for high-risk pregnancy, and family planning. In addition, patients can receive pap smears, cancer screening, mammography, ultrasounds, and STD prevention and screenings.

SWCHC provides medical care and mental health services at all six sites. Dental hygiene services are provided at five of the six sites, Central High School being the exception. For a student to be eligible to be seen by SBHC staff a student's parent or legal guardian must complete a signed consent form for enrollment at the school.

Details: This site concentrates on behavioral health treatment including psychiatric evaluation, treatment, and therapy for children and adults. Clinicians provide individual, family, and group counseling. Patients experiencing substance abuse can participate in the Intensive Substance Abuse and Mental Health program and seek continuing care as well.

Details: Our newest site in Bridgeport offers a full service that includes internal medicine where a patient can receive urgent care and same day appointments, women’s health, dental, behavioral health, pediatrics, and HIV/AIDS testing and treatment.
