Details: We began in 2002 as one primary care practice in Fitchburg providing medical services to the underserved. Community Health Connections was recognized as the place to find compassionate care, regardless of income or insurance status. We have greatly expanded in reach and scope to offer a wide range of services and care for individuals and families of all ages and incomes. Accessibility, compassion and quality remain at our core. Community Health Connections accepts most commercial insurance plans, MassHealth and Health Safety Net. A Sliding Fee Discount Program is available for those who qualify. Community Health Connections is a federally-qualified health center and licensed by the Massachusetts Department of Public Health.
Details: The mission of Edward M. Kennedy Community Health Center (Kennedy CHC) is to “help people live healthier lives.” We believe that everyone has a right to accessible, high quality, comprehensive, integrated and compassionate health care. As a community health center, Kennedy CHC has always provided comprehensive care to its patients, who are at disparate risk for chronic disease and poor health outcomes.
Details: The Kennedy CHC’s school-based health centers offer health services for children and teens on site at their schools. Health care is provided for students who have a primary care provider in the community as well as for those who use the school-based health center for their primary care. Our nurse practitioners and physician assistants collaborate and coordinate with school nurses and providers in the community. When school is not open, services are available at our health center sites in Worcester and Framingham for these students.
Details: We provide dental hygiene services (oral assessment, dental radiographs, dental hygiene treatment, fluoride treatments, oral self care instruction and health promotion). Quinsigamond students, faculty, staff and the general public are invited to call for appointments.

Details: Family Health Center of Worcester and the Worcester Public Schools have been partners in the school health centers for over 20 years. The school health centers offer a comprehensive array of preventive and primary care services in the location where children spend most of their time: in the school. The school health centers provide students with easy access to quality health care. They are open during regular school hours. Health care is provided by a licensed nurse practitioner working in conjunction with physicians and other resources from Family Health Center.
