Clinic Details:Since 2000, the Roosevelt Health Center has been working to improve adolescent health and well-being. We provide access to comprehensive and high-quality health care services including primary medical and dental care, behavioral health services, and health education in a respectful & youth-centered environment. Services are free of cost to all students. Roosevelt Health Center is operated by La Clínica, in collaboration with Oakland Unified School District, Asian Community Mental Health Services, Alameda County Health Care Services Agency - School Health Services Coalition, and Elev8/Safe Passages Partnership.

Details: Street Level Health Project promotes self-sufficiency for marginalized people of color by creating equitable and dignified access to health and employment regardless of socioeconomic or immigration status. SLHP engages community in constructing collective power and leadership in order to advance a more just, inclusive, and empathetic society. Street Level Health Project is an Oakland-based community center dedicated to improving the wellbeing of underinsured, uninsured, and recently arrived immigrants in Alameda County.
Details: The San Antonio Neighborhood Health Center is located in the midst of the East Lake District, one of the most multi-cultural districts in Oakland. Residents from Mexico, Central America, Ethiopia, Eritrea, China, Hong Kong, Vietnam, and many other countries come to SANHC for comprehensive medical care services. Staff speak over 12 different languages in order to serve patients in their preferred language. SANHC expanded in 2007, almost doubling in size. The health center now serves approximately 12,000 patients including children, teens, young adults, pregnant women, older adults and patients with chronic illnesses such as diabetes and hypertension.
Details: Founded in 2008, Roots Community Health Center is a 501(c)(3) non-profit organization dedicated to improving the status of health of East Bay residents. Roots implements its mission by providing high-quality, comprehensive and culturally appropriate health care, by implementing direct service models to increase access to care and thereby prevent the need for emergency services, and by coordinating cross-agency liaisons to ensure a more efficient, continuum of care in East Oakland. Roots Community Health Center was founded to address the growing need for accessible, culturally appropriate, community-responsive, comprehensive health care in Oakland, California. Specifically, Roots aims to eliminate wide health disparities and improve health outcomes for uninsured and low-income residents of East Oakland.
Details: Highland Wellness is part of Alameda Health System’s (AHS) network of community-based ambulatory clinics that expand access to primary care and AHS medical specialties. With the Adult Medicine Residency Program, Highland Wellness is also a leading medical training provider.
Details: In 1992 La Clínica, in collaboration with the East Bay Agency for Children and the Oakland Unified School District, opened the Hawthorne Health Center as one of the first school-based health centers in Alameda County. We serve students from World and Achieve Academies (formerly Hawthorne Elementary), Urban Promise Academy (UPA) Middle School, and children from the wider Fruitvale community. We offer primary care medical services, health education, and behavioral health services. We see patients from 3-18 years old and all services are provided without charge to students.
Details: La Clínica's first dental site opened its doors in 1978. Fruitvale Dental located in Oakland's Fruitvale District offers dental care to low-income families. Our doctors are trained to provide services to patients of all ages, from babies to the elderly. We provide a wide scope of care to all of our patients, including preventive care (exams, cleaning, polishing, hygiene, fluoride, sealant treatment), general dentistry, pedodontics, periodontics, prosthodontics, endodontics and orthodontics.
Details: Since 1973 Community Health Education (CHE) has enhanced the lives of the diverse communities we serve by providing programs and services that focus on the social determinants of health in order to enhance overall wellness and disease prevention. People free of preventable diseases live longer, healthier lives, and benefit the entire health care system by reducing health care costs. The CHE team comprised of more than 25 staff and over 60 active community promotores, volunteers and peers value Innovation, Empowerment and Collaboration to increase health access, overall well-being and disease prevention.
Details: With fewer and fewer public services available for behavioral health problems, La Clínica Community Behavioral Health Department, Casa del Sol, has played an important role in helping Spanish speakers who would not otherwise receive counseling or crisis intervention services. Casa del Sol was founded in 1973 and currently offers behavioral health services for Alameda County residents through a variety of programs that are funded to provide treatment to Alameda County's Spanish speaking communities. All clinical staff at Casa del Sol are bilingual in Spanish/English. Services are provided in a culturally focused manner centered on individual, family and community strengths. Evidence based practices are utilized to support resiliency, wellness and recovery.
Details: Our teen health center, Clinica Alta Vista (CAV), specializes in providing comprehensive health care services for adolescents and young adults. All of the staff and providers at CAV are committed to helping young people make informed decisions about their health and all are bilingual in Spanish and English. We offer comprehensive medical care, family planning, emergency contraception, prevention and treatment of STI's, including oral testing for HIV with results available in 20 minutes. All patients have access to our in-house social services, health education and nutrition counseling. For pregnant patients we provide options counseling, prenatal, postpartum and pediatric care. Through our Centering Program we give vital support to young parents including a scholarship program for those parents who stay in school.
Details: Youth Services stems from the enthusiasm of students participating in Casa En Las Escuelas Program, who when the school year ended, wanted to continue their work with CASA CHE. That's when Peer Health Education internships were created for students who wanted to learn Leadership and Community Action Skills, and participate in various community-based activities under the guidance of CASA CHE Health Educators. They also assist in outreach at La Clinica events, conduct Peer Education Sessions in schools, Needs Assessments and Community Action Plans. Casa CHE continues to educate and help build leaders within our communities.
