When a person walks through the doors of the CCC Recovery Center, they have taken that first step. They begin by learning about the disease of addiction and the journey to recovery. They learn how to move beyond the moment of crisis, undergo treatment and achieve long term sobriety. CCC Recovery Center uses a multi-disciplinary approach and evidence based practices to provide the appropriate combination of medical care, mental health counseling, education, peer mentoring and supportive housing for each individual. The Recovery Center also offers a culturally-specific addiction treatment service to African American clients. With all the necessary resources under one roof, Recovery Center staff can work with different providers and agencies to provide a continuum of care. CCC certified Alcohol & Drug Counselors are highly trained and committed to successful treatment and outcomes. Many on our counseling staff are in recovery themselves, bringing a unique and profound level of understanding to every interaction.     • Details: Some homeless individuals have problems that seem intractable – debilitating physical, mental and addiction issues that keep them in a cycle of homelessness. Central City Concern understands that the approach to the problems that afflict homeless people must be comprehensive. The Community Engagement Program (CEP) is a multidisciplinary recovery model designed to meet the needs of chronically homeless individuals with co-occurring mental health, addiction disorders and/or physical concerns. The initial goal for a person entering the CEP program is to help them obtain safe housing. Once they have a safe place to live, participants can begin to take the necessary steps toward recovery. CEP staff partner with physicians, mental health practitioners, social workers and vocational counselors to create a plan for each participant. Case managers continue to stay active with individual’s housing needs, connecting them to housing that is treatment oriented and eventually to secure permanent housing. The staff-participant ratio is 1-15. The team of care givers checks in regularly with each client so responses are concerted, timely and provide a continuity of care which is best for each individual. The CEP model is proven to be most successful in helping the homeless in their recovery efforts and in helping them to pursue full, productive lives.

Hooper Detox services are designed to assist with complex care needs and special populations. We welcome people seeking withdrawal management with the following care needs: Pregnancy; Benzodiazapines; Opiates, alcohol, and/or stimulants; Methadone; Diabetes, hypertension, or other medical conditions. Many of the staff at Hooper are in recovery themselves and have experienced the challenges that come with withdrawal and early stabilization. With enhanced resources, CCC was able to establish the Imani Center. The name Imani, meaning “faith” in Swahili, was chosen to remind people to have faith in themselves. The Imani Center provides culturally specific and responsive Afrocentric approaches to mental health and addictions treatment, peer support and case management. The services at Imani also create honor within the participant, our services and the agency. The Center also connects program clients to housing, employment and benefits support, and natural support systems within their own cultural community. The Imani Center consists of African American leadership and staff who provide group and individual supports, and pro-social activities in a culturally safe environment. This allows participants to authentically engage and address barriers that have previously limited their physical, emotional, spiritual and economic wellness. While the Imani Center is not a housing program, we support clients who need safe and appropriate housing by providing referrals to those individuals who are actively enrolled and engaged in our services.

Surrounded by role models and others who are succeeding in their recovery, the women see what life can be. The Letty Owings Center helps give them the tools they need to get there. The women become part of a larger community when they attend recovery support groups. Their children also become part of the larger community when they attend pre- school programs with youngsters from a wide range of income levels and family backgrounds. Individual and family counseling helps mothers and children learn to problem solve and communicate, laying the foundation for independent living. Women gain a feeling of accomplishment, giving them the confidence they need to begin rebuilding their lives, and their families. At Letty Owings young mothers become clean, sober and hopeful – able to reconnect with their children who might otherwise be placed in foster care.

There, creative staff members are crafting a new model of care - one in which people recover lives lost to their illness, and become active members of a community. A state licensed outpatient mental health and addiction treatment program for adults, Old Town Recovery Center has been a mainstay provider to Portland’s severely mentally ill for more than 20 years. The majority of clients experience both mental illness and addiction challenges, some are homeless, and all have been impacted by many years of poverty. Services in all program areas include: case management, counseling, addiction treatment, co-occurring disorder treatment, access to housing and eviction prevention, medication prescribing and management and access to medical professionals. The Old Town Recovery Center has more than 20 qualified mental health professionals on staff. In addition, the Center employs licensed medical providers to oversee medication management. On site is the Benefits & Entitlements program (BEST) to help people access the Supplemental Security Income (SSI) and Social Security Disability Insurance (SSDI) benefits to which they may be entitled. When Latino families move to Oregon, they not only leave behind their relatives and friends but also their community and support networks. Central City Concern developed the culturally-specific Puentes program to support Latinos in recovery in 2005. Puentes uses a multidisciplinary approach to provide alcohol and drug treatment and mental health care to individuals and to the entire family in a way that mitigates stigma and fear. Puentes staff are bilingual and bicultural; they understand Latino values, family roles and community structures helping Latino clients gain insight into cultural influences on behavior. Puentes clients receive primary health care, a recovery mentor, a mentor for the entire family and a case manager to help with housing and follow up care. With treatment, support and shelter, the family or individual can move from crisis to stability and reunification.

Central City Concern has partnered with the hospital system to create Recuperative Care (RC) so that these patients can be discharged to CCC housing. On-site staff are available to provide case management. Recuperative Care provides immediate housing, intensive case management and access to primary care at our Old Town Clinic. Patients are picked up from the hospital, given their own room and are immediately established with a primary care provider and case management team. Case managers check in daily with their clients, take them to follow up appointments and assist them with their daily needs. Additional addiction and mental health care services are available at the Old Town Clinic. Once clients are stable, they can focus on rebuilding their lives. Central City Concern specialists can help them get supportive housing, training, employment and the resources they need to recover and to become self sufficient. Recuperative Care staff works closely with Portland Area hospitals and CareOregon to identify patients in need of housing and case management.
