It began as two separate sites in 1992 and 1994 both located within nearby public housing communities, and then eventually merged, was renamed and moved to its current space on the East Falls/Germantown line. Abbottsford Falls provides comprehensive healthcare services, including Primary Care with integrated Behavioral Health, Outpatient Behavioral Health, Dental Care, Social Work, Outreach & Enrollment, Diabetes and Nutrition education and support, Mind/Body practices, prenatal care, medical specialty services, physical therapy, on-site pharmacy, transportation assistance and a fitness room. It underwent a renovation and expansion in 2007 in order to increase its capacity to serve even more patients. The site has a longstanding community advisory board whose composition includes some of the Network’s longest standing, first patients.

Formerly known as QCare, the clinic opened in 2013 and sees patients on a walk-in basis for minor or acute injuries or issues including physical exams, skin conditions, family planning, vaccines, diagnostic procedures, allergies, cold/flu and more. The clinic’s staff also provide assistance with health insurance enrollment, and can help patients get connected to a more comprehensive range of services at one of FPCN’s other sites. The clinic has some evening and weekend hours in order to be as accessible as possible to Philadelphia residents, helping people avoid the emergency room unnecessarily and receive care in a more appropriate setting for some of the aforementioned issues.

    • Details: The Stephen and Sandra Sheller 11th Street Family Health Services of Drexel University has been recognized as a national model of innovative care, by the Robert Wood Johnson Foundation’s LEAP project (Learning from Effective Ambulatory Practices), the Agency for Healthcare Research and Quality, and the American Academy of Nursing. 11th Street uses a transdisciplinary approach to deliver primary care, behavioral health, dental services, and health and wellness programs to more than 2,500 patients annually. The center’s health and wellness programs include diabetes and nutrition education, cooking classes, prenatal groups, Mind/Body practices, an on-site pharmacy, physical therapy, dance/movement therapy, and a fitness room with group exercise classes. Adults and children have a single point of access for care that addresses both the physiological and psycho-social aspects of the person and family. The Center’s target population is the residents of the four public housing developments in the 11th Street corridor. The site was originally operated by Drexel University School of Nursing and then joined with FPCN as the FQHC in 2002. FPCN manages many of the health center operations while Drexel faculty and staff are dedicated to bringing innovative and complementary programs to the center to benefit all patients.

Part of our funding is made available by the Health Resources and Services Administration (HRSA) through a grant. As an FQHC, we are able to provide our services regardless of a person’s ability to pay for the services. In cases where an individual lacks health insurance, we offer a discounted sliding fee program.
