Details: The Stacy Kirkpatrick House is a step-down respite facility, welcoming patients previously recovering from surgeries, long-term treatments, and other ailments at the Barbara McInnis House. The Stacy Kirkpatrick House - named after a beloved nurse (1964-2016) who was known for her infectious positivity and her unwavering compassion for our patients - offers 20 medical respite beds and is run by BHCHP nurses and operations staff. BHCHP’s step down model allows for medically frail patients to transition from round-the-clock care at the Barbara McInnis House to low-risk care at the Stacy Kirkpatrick House to long term housing facilitated by our case managers on staff.

Details: The mission of the Upham's Corner Health Center consists of a commitment to provide high quality, low cost, culturally sensitive, community-based health and social services to the residents of North Dorchester and adjacent neighborhoods. The Upham's Corner Health Center is a non-profit, organization. UCHC's IRS FORM 990 information is posted on the Web at www.guidestar.org. The Upham's Corner Health Center is organized to provide comprehensive, personalized and CONTINUOUS health services. The goal of the health center is to provide each patient with primary care in comfort and convenience. Each registered patient is assigned a personal primary care provider and a clinical care team. The provider and care team work together to provide ongoing care and coordinate specialty services and inpatient care as required. The health center physicians follow patients in need of hospitalization and participates in treating them in the hospital setting. Boston Medical Center provides the major hospital support for specialty consultation and inpatient care. UCHC's medical records and communication flow are maintained between the health center and hospitals. Medical records are filed by family to facilitate family-centered coordination of services.

Think of Whittier as a one-stop shop where you can come for your primary care, wellness programs, eye and dental care, and behavioral health services. Urgent care is available whenever the Clinic is open and pediatric patients will always be seen - even without an appointment.

Details: Codman's School-Based Health Center is a satellite clinic of Codman Square Health Center open Monday through Friday at the TechBoston Academy at 9 Peacevale Road. To become a Health Center member, students need to have a consent form signed. All Health Center services are confidential, and students need a pass from class to be seen.
