- Clinic Details: Respite care, known as rest and recovery, is available to homeless adults who are too ill to manage on the street or in a shelter, but not sick enough for hospitalization. Respite care is a short-term (10-14 days) opportunity for patients to recover from illness or injury in a safe place that provides nourishment, nursing care, medication instruction and other services. Intensive case management is provided, including linkage with job training, addiction and mental health treatment. Housing placement specialists are also available to offer assistance, especially with applying for benefits. Transitional Housing program offers the opportunity for 24 homeless men and women to live in a sober living environment for up to 24 months while receiving intensive case management services to address issues of substance abuse, mental health, health concerns, education, training, employment and accessing income benefits and permanent housing. The program emphasizes the strengths and positive attributes of every individual and providing an opportunity for success and making inroads into ending chronic their homelessness.
