Details: The Moorpark outpatient clinic is one of 11 operated county-wide by Santa Clara Valley Medical Center (SCVMC), and opened its doors in 1995. Here, you’ll find the same high-quality care and coordination available at all SCVMC locations. Our friendly and highly-qualified doctors, nurses, and staff offer a wide variety of services, including adult urgent/Express care, family medicine, pregnancy care, lab, radiology/diagnostic imaging, convenient on-site pharmacy, and more.

Individuals, many of whom do not speak English, have come to rely upon the Health Center’s multilingual team for comprehensive primary and preventative health care services. Details: Gardner Family Health Network’s Health Care for the Homeless (HCH) program continues to maintain its excellent relationships with community resource agencies in Santa Clara County and develops new relationships as community services develop and the needs of homeless people change. We have agreements to deliver health services during hours that homeless people are present at key homeless shelters and service sites from Palo Alto to Gilroy, including EHC/LifeBuilders, InnVision, St Joseph’s, Mountain View Day Worker Center, Bill Wilson Center, Georgia Travis Center, Salvation Army and other locations. Because of the high concentration of the homeless and homeless service sites in downtown San Jose, the new Gardner Downtown Health Center is expected to increasing the number of homeless we serve from 3,000 to 4,000 individuals and increase the number of clinic visits from 10,000 to 13,000 for individuals and family members who happen to be homeless.
