Clinic Details:Clinica Sierra Vista, a non-profit corporation, was created to provide health care to poor, migrant farmworkers who harvest the lush agricultural fields in the southern San Joaquin Valley. Today, Clinica Sierra Vista serves a geographically dispersed, low-moderate-fixed income, ethnically diverse, frontier-rural-urban-migrant-homeless patient population. It has grown to be one of the largest comprehensive migrant/community health center systems in the state.

Details: Our main purpose as a Federally Qualified Health Center is to enhance the provision of primary care services in underserved urban and rural communities. Back in 1973, concerned citizens from Kerman, San Joaquin, and surrounding communities worked together to form an organization that could bring quality healthcare to this underserved region. The dream was realized when our first primary care center opened in the town of San Joaquin to meet the medical needs of the residents of the rural western Fresno County area. Valley Health Team has grown steadily to meet the mounting healthcare needs of the region. Our leadership is governed by an active and involved board of directors, some of whom are patients of Valley Health Team. Services are provided by a care team of qualified professionals working in multi-disciplinary teams. Board certified physicians, certified physician assistants, psychologists, nurse practitioners, licensed clinical social workers and health educators, dentists, optometrists, and support staff work with leadership to provide culturally relevant, outcome-based care. Valley Health Team is a Joint Commission-accredited, private, non-profit Federally Qualified Health Center serving patients throughout the central California valley with an operating budget in excess of $15 million.
Details: Fillmore Family Medical Group in Fillmore, California is a Federally Qualified Health Center (FQHC) affiliated with Ventura County Medical Center (VCMC), part of the Ventura County Health Care Agency (VCHCA). Ventura County is a beautiful, family-oriented community with excellent schools and multiple cultural and outdoor activities including surfing, hiking, mountain-biking and kayaking.
Details: Central City Community Health Center (CCCHC) Inc., is a 501 (c) (3) non-profit, Section-330 Federally Qualified Health Center. CCCHC was established in 1994 to serve low-income and medically underserved residents that fall below the 200-percent Federal Poverty Level.
Details: Lyon-Martin Health Services provides excellent health care to heterosexual women, bisexual women, lesbians and transgender people in a safe and compassionate environment, with sensitivity to sexual orientation and gender identity; all services are regardless of ability to pay. Not a walk-in clinc. Appointment must be made ahead of time and you have to establish yourself as a patient first.
