Details: Peninsula HealthCare Connection (PHC) promotes integrated primary and mental healthcare and intensive case management for homeless individuals or those at risk of becoming homeless in Santa Clara County, regardless of their ability to pay. We will collaborate with local hospitals, healthcare providers, social service agencies, and similarly committed partner agencies to this end. Peninsula HealthCare Connection (PHC) was founded in 2005 and is the only fully licensed medical facility providing free, direct healthcare services to homeless and low-income residents of North Santa Clara County. The clinic is located within the Opportunity Center in Palo Alto, and is staffed in part by Palo Alto Medical Foundation Medical Doctors.

We have agreements to deliver health services during hours that homeless people are present at key homeless shelters and service sites from Palo Alto to Gilroy, including EHC/LifeBuilders, InnVision, St Joseph’s, Mountain View Day Worker Center, Bill Wilson Center, Georgia Travis Center, Salvation Army and other locations. Because of the high concentration of the homeless and homeless service sites in downtown San Jose, the new Gardner Downtown Health Center is expected to increasing the number of homeless we serve from 3,000 to 4,000 individuals and increase the number of clinic visits from 10,000 to 13,000 for individuals and family members who happen to be homeless.
