Our Program of All-Inclusive Care for the Elderly (PACE) is a health plan designed for seniors with delicate health care needs. PACE provides participants with complete medical, social, nutritional, rehabilitative and transportation services, making it possible for seniors to live with dignity and pride in their own homes while receiving the care they need. If someone in your family is 55 years or older and has chronic medical conditions, PACE may be the right option for them. Our program serves seniors who: Are 55 years or older; Live in the PACE service area; Need assistance with daily living; Require a level of care equal to nursing home services; Can live safely in the community with the help of PACE services.
