The mission of the Community Health Centers of Lane County, a federally qualified health center (FQHC), is to improve the health and wellness of our community through access to affordable, holistic healthcare. We have adopted a model of primary care called Patient Centered Medical Home. This model of primary care prioritizes the patient, who has increased access to other services  beyond general medical care at our clinics. We coordinate our patients' health care across the community, among specialists, emergency and hospital visits, diagnostic testing services, and other services. Patients at Community Health Centers of Lane County are assigned to a care team, consisting of many different types of professionals. Each team member adds different skills and knowledge to support the health of our patients. We believe healthy people are the foundation of healthy communities.
