The mission of the Community Health Centers of Lane County, a federally qualified health center (FQHC), is to improve the health and wellness of our community through access to affordable, holistic healthcare. We have adopted a model of primary care called Patient Centered Medical Home. This model of primary care prioritizes the patient, who has increased access to other services  beyond general medical care at our clinics. We coordinate our patients' health care across the community, among specialists, emergency and hospital visits, diagnostic testing services, and other services.
