San Luis Health Center is located in the oldest town in the state of Colorado, San Luis (established in 1851), and has been managed by Valley Wide Health Systems, Inc. since 1977. The clinic has undergone various changes throughout the years, ranging from relocating within the town to updating its health records to an electronic format. In 2004, San Luis Health Center (SLHC) relocated from HWY 159, to its new location at the Costilla County Public Health building, across the parking lot from the Emergency Medical Services building. In addition to changing locations, we’ve added many technological updates, which enhance our patient’s care outcomes. We provide a safe and comforting atmosphere, dedicated to providing excellent patient care to the rural population in the town of San Luis and surrounding areas such as Costilla County, the towns of Blanca and Fort Garland and Northern New Mexico. Our clinic serves patients of various backgrounds and socio-economic statuses, including: low-income families and offers specialized assistance for our Migrant patients. SLHC operates Monday-Friday from 8:00-5:00p.m., with co-located Behavioral Health services also available on Wednesdays and Fridays to provide counseling needs to all patients. A full range of Chronic Care services are offered, such as asthma care, to provide literature and educational material so you can fully understand how to successfully manage your condition. We also offer both childhood and adult vaccinations. Ernesto Pacheco Dental Clinic is also located within the building to provide all of your dental needs for you and your family. Family Health & Planning services are available to provide you with options based upon individual beliefs and needs. Our clinic also offers FREE HIV screening services and a full spectrum of STD testing and treatment in a safe and confidential setting. Our doctors, physician assistants, nurse practitioners and physical therapists are dedicated to making sure all Valley-Wide patients receive individualized patient care, in a respectful and inclusive manner. We focus our efforts on providing excellent service to making sure our patients feel comfortable and continue to return to our clinic for their individual medical care. We treat our patients as individuals and value their safety, confidentiality and health-care needs. At SLHC, we take a proactive approach to your healthcare. We are one of two clinics to implement a follow-up appointment log. This ensures that all patients receive a call back from one of our schedulers for chronic conditions such as diabetes. This allows us to schedule and confirm future appointments, providing clarity and continuity of on-going care.
