During the warmer months, CHP on Wheels offers routine physicals, wellness visits, chronic disease management and sick visits on our mobile health unit - a recreational vehicle that has been adapted for health care. CHP on Wheels travels to some of South County’s more remote towns each month with a family nurse practitioner on board who can provide care. CHP on Wheels offers free health screenings, health education and enrollment for MassHealth, SNA (formerly food stamps) and the WIC (Women, Infants and Children) nutrition program. CHP on Wheels is one of the few mobile medical units in the state that offers physicals, disease management, acute care and health assessments. Our mobile van is also adapted for use by CHP’s ophthalmology (vision care) specialist. By traveling to small towns and villages - far from health centers and public transportation, CHP on Wheels removes barriers to care and improves the health of our region’s most vulnerable residents. It plays an essential role in CHP’s success in connecting hard-to-reach populations with health care. CHP on Wheels opens its door in rural settings from town halls, libraries, Councils on Aging, subsidized housing and food pantries. Thanks to the Massachusetts Medical Society and Alliance Charitable Foundation, Community Health Programs has set up an exam room for ophthalmology (eye care) on our mobile outreach van. We offer screening, diagnosis and treatment for most eye diseases at three of our primary care practice sites. If you haven’t had an eye exam in more than a year, now’s the time!
