Primary Care Services at Community Healthlink offer easily accessible, coordinated, recovery-oriented primary health care on-site, for people who have difficulty accessing their Primary Care Provider, or who are not receiving their primary health care elsewhere. Because Primary Care Services are integrated with Community Healthlink’s other outpatient clinic services, clients can access primary health care while receiving mental health treatment, case management or other services. Primary and behavioral health care management offered by clinic staff accommodates individual client needs. Every client receives care coordination, and care support for practical and social needs. For those with more complex, high-risk needs, Primary Care Services will provide intensive care management.
