The Long Island FQHC is bringing a new kind of care to the communities that need it most.  Our goal is to see that every Long Islander has a "medical home" - a place where people you know provide the kind of primary and preventative care that safeguards the health for you and your family.  
Great care delivered by compassionate doctors and nurses are available right in your own neighborhood.   Our six health center locations provide multiple health services such as  Family Medicine, Internal Medicine, Pediatrics, OB/GYN/ Dentistry, Podiatry, Optical, Behavioral Health and Nutrition. We can even help you obtain low or no-cost insurance coverage, if eligible.

•	Public health insurance (Medicare, Medicaid, Child Health Plus).
•	Co-payments and deductibles are due at the time of service.

•	Sliding Scale Discount Program for reduced rates based on your family’s size and income for qualified income-eligible insured, uninsured and under-insured patients. Apply for our program during your visit or print and complete the form before your visit:
