We believe in caring for the whole person. The Willow Pass Wellness Center was conceived as a collaborative effort to integrate behavioral health and primary care for county mental health patients. We serve central county residents being seen at any Contra Costa Health Services adult mental health clinic, but primarily work with patients from the Central Adult Mental Health Clinic, which is located in the same building as the Wellness Center. We are a team of physicians, primary care providers, nurses, mental health case managers, community support workers, and administrative staff, all dedicated to providing the best possible care for our patients. We seek to improve access to primary care for patients with mental health needs by having primary care services located in the same place and by assisting with transportation. Our collaborative efforts work to provide the best care for patients.
