The Wellness Coalition works with our community partners to coordinate services and share information to increase quality, efficiency and effectiveness of health care services. We focus on improving access to health care for persons with chronic diseases and limited or no health insurance in Central Alabama. We provide health care access services primarily to the River Region’s non-elderly adults who have chronic diseases (diabetes, heart disease, asthma, obesity and others) and no health insurance - an estimated 45,622 individuals (2010 census). We also provide, through service contracts with local agencies, services for persons of other ages, such as elderly and children or those who are underinsured; and we provide community health and wellness interventions in other venues such as churches and community agencies.
