In November 2015, Martin Community Health Center relocated to its current location on Pugh Street, which is centrally located in Martin. Horizon has been a part of the Martin community for over ten years and currently has four providers on staff. Providers and staff at Martin Community Health Center provide patients with a broad range of health services to meet the needs of individuals and families in their community. Martin Community Health Center accepts all insurance and offers reduced prices on office visits and all other services provided in the clinic to all individuals and families who qualify through the Sliding Fee Program.
