Details: The HOPE Project mobile clinic provides services to homeless people at six sites in southern and eastern Alameda County. These services include primary healthcare (adults, pediatric and women’s health), dental services (through the coordination of volunteer dentists), HIV/AIDS testing, prescriptions for uninsured, TB tests and immunizations. The HOPE Project was established in 1999 by Tri-City Health Center, Abode Services, the City of Fremont, Alameda County Behavioral Health Care Services, and local churches and community groups. Since the reasons for homelessness are complicated and varied, it takes the expertise of many different agencies, working collaboratively, to support these individuals. Providing assistance with medical care, housing, and case management via mobile clinic delivers “one-stop” health care and social services directly to the locations where the homeless congregate and yields a much higher success rate than when the same issues are dealt with separately. On average, the HOPE Project has 3,640 client visits and provides 5,827 services. The majority of clients sought out medical care, closely followed by behavioral health services, case management and housing assistance.

We have full-service school health centers at James Logan High School in Union City and Tennyson High School in Hayward. These health centers are a low-cost, safe and convenient place for any student to receive the medical and mental health care they need to succeed in school and become productive members of their community. A student who is tired, sick, suffering from asthma, stressed, depressed or scared often experiences academic performance issues as a result and an increased risk of dropping out of school. Much like our main community-based health centers, our school health centers advocate holistic health care, paying careful attention to the dynamic relationship between mind, body, and spirit. Our primary objective is to make important health and wellness information more readily accessible to youth and provide an effective tool for them to take control of their own well-being.

Details: Founded in 1971 the Tiburcio Vasquez Health Center has delivered culturally and linguistically appropriate services to residents of southern Alameda County for over 40 years. Last year, our clinics served nearly 15,000 clients, providing primary health care and mental health services to men, women and children of all ages.
