Clinic Details:When longer-term care becomes necessary, you’ll feel right at home with our staff of professional and attentive caregivers. Our extended care facility is staffed to care for residents 24 hours a day, seven days a week, supported by the full capabilities and facilities of Summersville Regional Medical Center hospital services. Our residents often come to us for care because we are a part of the hospital, and physician services are only steps away. We provide care for 52 residents covered by Medicare, Medicaid, and some private insurance providers. Nursing and support care is provided by our registered nurses, licensed practical nurses, certified nursing assistants, activity professionals, and social workers. We offer a range of rehabilitation services, including physical therapy, occupational therapy, speech therapy, and respiratory therapy. A visiting dentist, optometrist, and audiologist also provide care, as well as physicians who specialize in orthopedics, urology, podiatry, cardiology, and general surgery. When it is time for you or your family member to receive skilled or long-term care, we want to be your care provider. Details: Ritchie Regional Health Center (RRHC) was established in 1994 as part of a pilot school based health center program. Based in Ritchie County High School, RRHC offered health care services to students, staff, and teachers. Today, RRHC offers primary care services at seven campuses across Doddridge, Pleasants, Ritchie, and Wood counties. In addition to being primary care centers, five of RRHC’s campuses are also designated as School Based Health Centers (SBHC).These clinics are located within school grounds and can support the schools (Teachers, Students, Staff); however, they are also open to provide clinical services to the student’s parents and the community-at-large. Ritchie Regional Health Center is designated at a Federally Qualified Health Center and as such is directed by a board comprised of patients and community members.
Details: Community Care of Clay offers an array of medical services including comprehensive primary care and chronic disease management and walk-in acute care for non-chronic and non-life-threatening illnesses and injuries. This location also offers an in-house pharmacy and a wellness center. Community Care of Clay has received a Level 3 accreditation - the highest possible - from the National Committee for Quality Assurance (NCQA) for providing exceptional service and high quality care. The CCWV chronic disease management model provides for a complete range of on-site services for comprehensive patient care. The mix of CCWV’s medical providers and nurses provide assessment and education so patients (and their families) better understand their illnesses and what they can do to help themselves, including any referrals for additional care that may be necessary. CCWV’s Care Managers and the patient’s medical provider work closely together to give the patient the best care management possible. As part of this expanded care management assessment, the health history of all patients is reviewed to assure that each has been screened for chronic conditions including high blood pressure, high cholesterol, depression, weight management, physical activity, and asthma. Other risk factors, such as pregnancy, co-morbid conditions, or family history of disease, are also assessed. CCWV offers family planning services and participates in BCCSP. For children, immunization are available through the VFC program and private insurances. Our state-of-the-art Electronic Health Record (EHR) and a team of healthcare professionals are focused on chronic disease management and quality care outcomes throughout their system of Community Health Centers. Our EHR enables the medical providers to quickly access and manage clinical information such as medical history, progress notes, laboratory results, diagnostic imaging, medication orders, and clinical consultations. Most importantly, the EHR provides key reminders and tracking measures that allow the providers to monitor the patients’ health in order to detect small problems as they arise and provide preventive services on the front end.
