There are approximately 1,809 people experiencing homelessness in the county and the program provided essential health care to 2,215 people in 2018. Access to health care is provided through coordinated outreach integrated with an elaborate and effective clinic system. Our system has three Health Care Centers located within homeless shelters, five Public Health Department Health Care Centers. The program collaborates with several other Federal Qualified Health Centers (FQHC) to ensure continuity of care and prioritizing limited resources. Dental services are provided through a collaboration with a neighboring FQHC. Medical outreach is an important method of removing barriers to health care for those who are experiencing homelessness. The field service team consists of Public Health Nurses (PHNs) stationed in approximately thirteen homeless shelters and transitional living centers. The nurses provide triage care and make referrals to the Health Care Centers and shelter-based clinics for those needing additional care. The Health Care for the Homeless (HCH) program in Santa Barbara County continues to excel as a provider of health care to people who are experiencing homelessness. The ongoing efforts of the team including Physicians, Dentists, Registered Nurses, Physicians Assistants, Nurse Practitioners, Administrators, Health Educators, and Medical Assistants ensure services that otherwise would be unavailable to the homeless community.

  • Sbnc - Goleta Neighborhood Clinic

    • Contact Phone: (844) 594-0343
    • Details: Santa Barbara Neighborhood Clinics (SBNC) has been a leader in community health since 1998, when Isla Vista Health Projects, Carrillo Medical Clinic, and Westside Medical Clinic joined forces. Our founding mission to provide healthcare services for all regardless of ability to pay was predicated on the philosophy that obtaining quality medical and dental care is a fundamental right for all people. This conviction was embraced by the founding doctors, and business and community leaders, they knew the lack of access to quality healthcare for all would have to be addressed by a unified effort.
