Malta House Of Care Mobile Medical Clinic

Details: Our mission is to provide free primary health care to the uninsured. We deliver that health care through operation of a mobile care vehicle that has a fixed schedule in four distinct neighborhoods across the City of Hartford. Coupled with coordinated communication efforts at schools, homeless shelters, parishes and churches, community outreach and ready access to those who are most in need is assured. The Malta House of Care Mobile Medical Clinic has been key in responding to the plight of the uninsured in the Greater Hartford CT region for the past five years. Through a dedicated cadre of physician, nurse and non-medical volunteers, the Malta House of Care Mobile Medical Clinic has provided over 25,000 patient visits over that time period with 2,200 individuals identifying Malta House of Care as their only medical home. We have received special recognition as a major source for the provision of safety net health services in the Greater Hartford Region. This is evidenced by the recognition we have received from the Hartford City Council and the Office of the State of Connecticut Healthcare Advocate, as well as being selected as Connecticut’s Ashoka Green Mountain Coffee Changemaker representing the best example of social responsibility and achievement. We are grateful for the strong financial and in-kind support we receive from all corners of our community. Most importantly, our impact is gauged by those who are most in need: through the profound gratitude of individual patients who now experience access to healthcare and the resulting positive health outcomes. Fairgate Community Health Center

Details: For more than 35 years Optimus has been an important part of serving the health care needs for individuals and families in Connecticut. We are at the center of your community providing you with the services you need most. Whether you have health insurance or not, we will be there for you. To improve the overall health of our communities in Bridgeport, Stamford, and Stratford, particularly the medically underserved, by providing preventive, primary care and supplemental health care services along with health education in a culturally sensitive manner regardless of ones ability to pay. Optimus Health Care is a mature Federally Qualified Health Care Center primarily funded by Health and Human Services. It is the largest provider of primary health care services in Southwestern Connecticut. With more than a dozen JCAHO accredited, comprehensive community health centers located in Bridgeport, Stratford, and Stamford, Optimus provides complete health care services, regardless of race, income and insurance status.

The Haven Free Clinic

Details: The HAVEN Free Clinic is a student-run primary care clinic partnered with Fair Haven Community Health Center (FHCHC) and Yale University. Our model is unique in its inclusion of students across the disciplines of medicine, nursing, physician’s associates, and public health. Since opening our doors in November 2005, we have conducted over 4000 adult patient visits, representing more than 450 unique patients. Patients are seen by teams of senior and junior students with guidance from faculty preceptors. Currently, we see an average of 20 patients per week. HAVEN operates Saturdays from 9a-12p out of the Fair Haven Community Health Center. Our programs are targeted to residents of the Fair Haven neighborhood. HAVEN Free Clinic serves uninsured adult patients in the Fair Haven neighborhood. In order to be a patient, you must: 1.) Live in the zip code 06513 2.) Be between the ages of 18 and 65 3.) Have no health insurance. 4.) Not already be a patient at Fair Haven Community Health Center

Details: KCC is a 501 (c)(3) non-profit free healthcare clinic founded in 2002 by Dr. Z. Michael Taweh of Newtown, CT, in memory of his son Kevin. KCC provides free medical services to persons 18 years of age and older, who are uninsured or underinsured and who meet certain income guidelines. At present, KCC covers the towns of Newtown, Botsford, Dodgingtown, Sandy Hook, Hawleyville, Roxbury and Bridgewater. Our Vision: Inspired by the need, our vision is to build an independent facility to: increase our capacity to see more patients; provide educational programs both to our patients and area residents; establish an outreach support center to improve and enrich the lives of all in our communities.
