Clinic Details:In 2011, PCC South Family Health Center was relocated to a newly renovated two-story, 18,500 square-foot building. The facility has achieved Leadership in Energy and Environmental Design (LEED) Gold certification from the U.S. Green Building Council. PCC South Family Health Center is the location of the Birth Center at PCC.

Details: Alivio Medical Center opened its doors to an eagerly awaiting community on January 4, 1989. On that day, the vision to create a community-based health center, responsive and culturally sensitive to many of Chicago’s most underserved and vulnerable communities, was finally realized. The Chicago neighborhoods of Pilsen, Little Village and Back of the Yards have been significant points of entry for immigrants and remain the home to a growing Mexican immigrant population in addition to an expanding community of Mexican Americans. Today, we serve families and individuals who historically have had limited access to healthcare for many reasons including: language and cultural barriers, intimidation of traditional healthcare institutions, limited awareness of available resources and/or fear of seeking help due to immigration status. Consequently, health promotion, education and prevention are integral parts of our comprehensive programs. Alivio’s bilingual, bicultural providers serve all patients regardless of ability to pay. Alivio Medical Center is a Federally Qualified Health Center and a grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).

Across the street from the health center is the PCC Austin Farm.
Details: PCC Walk-In Wellness Center at West Suburban Medical Center offers urgent care for illnesses and injuries that are not true emergencies. Patients can be seen without a scheduled appointment for a variety of health conditions. In addition, any patient that does not have a primary care provider can follow up with a primary care provider at one of our health centers and make PCC their medical home.This is also the location of our Chemical Dependency Clinic, Ultrasound Clinic, and Sports Medicine Clinic.
Details: PCC Erie Court Health Center is located within the professional office building of West Suburban Medical Center. PCC Erie Court provides obstetrics and gynecological services.
Details: PCC Community Wellness Center at West Suburban is now located on the 7th floor of West Suburban Medical Center's Professional Office Building. Patients can easily come to appointments for primary care and behavioral health care.
Details: Behavioral Health Services of Circle Family HealthCare Network provides services to adults and children who are diagnosed with serious mental illness, emotional disturbances, and/or have significant loss of normal functioning. Outpatient services are provided from our health center at 5002 W. Madison St. Residential care is provided for up to eight (8) CILA (Community Integrated Living Arrangement) adult clients through an off-site location as part of a special program.
