The Stephen and Sandra Sheller 11th Street Family Health Services of Drexel University has been recognized as a national model of innovative care, by the Robert Wood Johnson Foundation’s LEAP project (Learning from Effective Ambulatory Practices), the Agency for Healthcare Research and Quality, and the American Academy of Nursing. 11th Street uses a transdisciplinary approach to deliver primary care, behavioral health, dental services, and health and wellness programs to more than 2,500 patients annually. The center’s health and wellness programs include diabetes and nutrition education, cooking classes, prenatal groups, Mind/Body practices, an on-site pharmacy, physical therapy, dance/movement therapy, and a fitness room with group exercise classes. Adults and children have a single point of access for care that addresses both the physiological and psycho-social aspects of the person and family. The Center’s target population is the residents of the four public housing developments in the 11th Street corridor. The site was originally operated by Drexel University School of Nursing and then joined with FPCN as the FQHC in 2002. FPCN manages many of the health center operations while Drexel faculty and staff are dedicated to bringing innovative and complementary programs to the center to benefit all patients.
