Details: Open Pantry's Open Door Social Services program provides case management, housing search assistance, medical, mental health, and substance abuse referrals for homeless people living in area shelters, on the streets, or temporarily doubled up with friends or relatives. Open Door case managers provide a variety of services in relation to individualized service plans. They assist people in meeting basic needs, accessing entitlement benefits, connecting with vocational rehabilitation and employment programs, understanding letters they receive, filling out forms, sending for birth certificates. Open Door staff work in conjunction with Open Pantry's other programs and local agencies to provide comprehensive services for clients. Referrals are made so people can obtain shelter, permanent housing, health care, mental health and substance abuse treatment. The services provided by Open Door Social Services are essential in assisting people in need.
Details: We provide access to a fully licensed health clinic on site, a resource room with computer access and trainings, and case managers working with clients daily to remove barriers to permanent housing. Our Resource Center is handicap accessible, and we are conveniently located on a PVTA line.
Details: A nurse provides health care six mornings and two evenings a week for homeless people at 769 Worthington Street. Dental services for the homeless are also available two days a week at this location.

Details: Baystate Brightwood Health Center/Centro de Salud, a service of Baystate Medical Center, is a full service health center staffed by physicians, nurse practitioners, nurse-midwives, and other health care professionals. We provide comprehensive primary care for babies, children, and adults of all ages. As a patient, you have your own primary care provider - a physician or advance practitioner who works as part of a team to provide you with the best health care possible. In addition to primary care, our patients have on-site access to obstetrics/gynecological care provided by certified nurse-midwives, behavioral health services, an opioid addiction treatment program (suboxone), HIV medical care and support services, laboratory services, a pharmacy, and social services.
