The agency was formed with a MISSION to provide high quality primary healthcare and related services to the community regardless of ability to pay, in a manner which demonstrates in word and deed the love of Jesus Christ. Since receiving Federally Qualified Health Center (FQHC) status in 2001, CCHC has propelled an expansion of programs and services. While primary medical care is our anchor program, CCHC has become equally renowned for our presence as a multi-pronged social service agency. By incorporating primary medical treatment into a comprehensive slate of supportive programs, CCHC strives to meet the holistic needs of our community residents. CCHC currently operates seven sites throughout the Chicago land area, including four community health centers, as well as a mobile health van providing a wide range of primary medical and social services.

Details: This high school health center provides affordable, confidential, convenient, youth-centered primary and preventative health care for students who might otherwise not receive care. A family nurse practitioner and licensed clinical social worker provide primary medical and psychosocial services to all students whose parents have submitted signed consent forms. For parents or guardians, health care provided at school means they miss fewer days of work. For students, on-site health care can mean fewer missed days of school and decreased incidence of pregnancy, sexually transmitted disease, and substance abuse. The TCA Health satellite center also is sponsored by the Illinois Department of Human Services’ Bureau of Primary Health Care and Carver Military Academy.
