Clinic Details:SWCHC is located in six (6) Bridgeport city School Based Health Centers (SBHC): Bassick High School, Central High School, Blackham School, Read School, Roosevelt School and Cesar Batalla School. SWCHC provides medical care and mental health services at all six sites. Dental hygiene services are provided at five of the six sites, Central High School being the exception. For a student to be eligible to be seen by SBHC staff a student's parent or legal guardian must complete a signed consent form for enrollment at the school.

Details: Homeless patients and families can access all services at SWCHC, with additional interventions available to meet the specialized needs of this population. A team of clinicians and support staff visit 10 licensed shelter clinics on a rotating weekly schedule, with referrals for specialty care, and intensive medical follow-up performed at the sites as needed. In addition, the Homeless Team offers health care services to “street” homeless at non-traditional locations such as parks, bridges and feeding vans.
