Clinic Details:Our mission is to serve as the patient-centered medical home for our communities to achieve and maintain a positive state of wellness, particularly for the uninsured and underserved. OPTIMUS is a 501(c)(3) non-profit organization, supported through patient revenues, the generous support of the community, and grants from federal, state and local government. and private and public foundations. All patient revenues are used for the actual cost of health care.
Details: Homeless patients and families can access all services at SWCHC, with additional interventions available to meet the specialized needs of this population. A team of clinicians and support staff visit 10 licensed shelter clinics on a rotating weekly schedule, with referrals for specialty care, and intensive medical follow-up performed at the sites as needed. In addition, the Homeless Team offers health care services to “street” homeless at non-traditional locations such as parks, bridges and feeding vans.

Details: We provide services to previously incarcerated individuals in the greater Bridgeport community. Through the dedication of our Community Health Workers, we provide care and understanding of the concerns facing citizen returning to our community with chronic health needs.
