About this clinic

Abbottsford Falls is the oldest of FPCN’s locations. It began as two separate sites in 1992 and 1994 both located within nearby public housing communities, and then eventually merged, was renamed and moved to its current space on the East Falls/Germantown line. Abbottsford Falls provides comprehensive healthcare services, including Primary Care with integrated Behavioral Health, Outpatient Behavioral Health, Dental Care, Social Work, Outreach & Enrollment, Diabetes and Nutrition education and support, Mind/Body practices, prenatal care, medical specialty services, physical therapy, on-site pharmacy, transportation assistance and a fitness room. It underwent a renovation and expansion in 2007 in order to increase its capacity to serve even more patients. The site has a longstanding community advisory board whose composition includes some of the Network’s longest standing, first patients.
