ACCESS provides a continuum of care model that connects patients to health care resources both within and beyond the walls of our 35 federally-qualified health centers (FQHCs). ACCESS' services are designed to address the health of our underserved communities in such areas as preventive care, chronic disease management, and support services. To address patients' comprehensive health needs, ACCESS physicians, nurse practitioners, midwives and other providers are teamed with outreach staff, case managers, social workers and substance abuse counselors to advance a continuum of care.

Details: Our Touhy community health center was the first center we opened. A small but dedicated group of concerned residents, The Neighbors for a Healthy Rogers Park, were instrumental in opening our first site. They conducted a community health care assessment to show the need, got local leaders to support it, and partnered with us to bring a federally qualified health center to the neighborhood.
Details: Presence Saint Francis Hospital is a nationally recognized, full-service hospital with a Level I Trauma Center and is listed as one of the Best Hospitals in Metro Chicago by U.S. News & World Report. Recently, we were awarded Magnet Nursing designation for a second time by the American Nurses Credentialing Center's for excellence in nursing - the highest in national nurse credentialing.

Details: Erie Evanston/Skokie Health Center is Erie’s first location outside of Chicago. Erie Evanston/Skokie provides a wide variety of services for adults and children, including primary care, women’s health care, prenatal care, pediatric care and behavioral health care.
Details: Hayat Clinic was founded in September 2003 under its parent organization AM-RESCH Foundation. Our mission is to provide free healthcare to the uninsured and underinsured by all-volunteer Muslim physician staff and treat patients regardless of their ability to pay.
Details: Trilogy Behavioral Healthcare, Inc. serves people with serious mental illness in the Chicago area. Trilogy’s mission is to assist people in their recovery by helping them discover and reclaim their own capabilities and life direction. Services are designed to maximize independence and quality of life by helping people identify their own strengths and to develop their own wellness recovery plans. The Heartland Health Centers facility at Trilogy is fully integrated. Our primary care team works together with Trilogy’s mental health care team looking holistically at the needs of each patient and utilizing shared medical records. It is a nationally recognized program serving more than 900 people per year. In 2012, it won the National Council for Behavioral Health’s Reintegration Award for Clinical Medicine and the 2012 NAMI-IL Heroes Program Award for our Integrated Healthcare Program.
Details: Mercy to Mankind Clinic, NFP, is a volunteer-driven neighborhood clinic that partners with our providers and other organizations in an effort to address the health needs of our community. Guidance for our healthcare ministry is fulfilled though Rahmat E Alam Foundation, providing spiritual support for the community.
