About this clinic

All For Health, Health For All (AFH) Community Health Center is proud to have been delivering access to the highest quality health and human services to the medically underserved and indigent communities in the County of Los Angeles for over twenty years. AFH is a California Nonprofit Public Benefit Corporation with a Federally Qualified Health Center [FQHC] designation under Section 330 of the Public Health Service Act; it is also a contracted partner with the Los Angeles Department of Health Services. Since it initially opened its doors in 1990 as Broadway Family Medical Center, All For Health, Health For All has grown and consistently reinvented itself to meet the ever-changing needs of the community. With seven locations presently operating in three different cities, our services have expanded from general family practice including pediatric care to mental and behavioral health, as well as adult day health ca¬¬re services. All For Health is proud of the invaluable services that we have been able offer to our community and we are always striving to raise the bar even higher by reinventing the role of community clinics for the betterment of our patients’ lives. As a community-based provider of quality health care services, we recognize and respect the importance of our community’s health care needs. At AFH, our patients always come first. Our medical model is designed to deliver patient-centered care through an integrated delivery system for the whole family. Our systems are state of the art, and we utilize modern equipment, such as Electronic Health Records, that allows us to provide optimum care for the patient and their medical needs. Moreover, our clinic makes thorough use of the patient-physician relationship and enforces the integrative medicine model, which on wellness and the maintenance of good health. Our patients are more than just users of our services; they are apart of AFH’s governance structure. More than fifty-one percent of our Board of Directors is made up of current AFH patients. AFH prides itself on the quality of care delivered by its premier health care professionals.

Is this clinic actually free?
Often not entirely. "Free clinic" usually means one of three things: a volunteer clinic with no charge, a community health center with a sliding fee scale based on household size and income, or a clinic that charges a small flat visit fee. A sliding scale can still leave a bill. Ask what the lowest tier costs before you book.
What if I have no income at all?
Most sliding-scale programs have a lowest tier for people with no income, and many accept a signed self-declaration when you have no pay stubs to show. Ask for the clinic's discount program by name and ask what proof they take. Being told "bring proof of income" does not mean you are turned away for having none.
What should I bring to a first visit?
Usually a photo ID, something showing your address, proof of income or a self-declaration, your insurance or Medicaid card if you have one, and a written list of your current medications and doses. Call ahead — requirements vary, and arriving without the right paperwork is the most common reason a first visit gets rescheduled.
Do I need insurance, or immigration documents?
Federally funded health centers serve patients regardless of insurance status and do not require proof of citizenship to provide care. Volunteer and charitable clinics set their own rules. If it matters to you, ask before your visit what they require and what they record.
Is dental care included?
Usually not, and this is the single most common disappointment. Dental is frequently a separate program with its own eligibility, its own waiting list, and a narrower service list — extractions and urgent pain may be covered while fillings, crowns, root canals, and dentures are not. Confirm that dental is offered at this specific location before travelling.
Can I get a prescription refilled here?
Most clinics require an appointment before they will refill anything, and a first appointment can be weeks out. If you are close to running out of a medication you cannot safely stop, say so when you call — say the medication name and how many days you have left. Ask your pharmacy at the same time; pharmacists can sometimes authorise a short emergency supply.
What if I need care today?

For something urgent but not an emergency, an urgent care clinic or a hospital emergency department must evaluate you regardless of ability to pay.
