About this clinic

Annex West Health Center is operated as a partnership between Action Wellness (formerly Action AIDS) and FPCN. The health center opened in January 2015 and is based on the patient-centered medical home model, which utilizes a team approach to foster communication between provid­ers and patients, increase care coordination, and enhance overall quality of care. At Annex West, FPCN Nurse Practitioners work with Action Wellness med­ical Case Managers to provide services including: ongoing individualized care tailored to the patient’s needs, access to a behavioral health consultant, linkage to ongoing behavioral health services, vocational services, and housing information services. Annex West has a robust linkage with prison systems in order to serve people reentering society. In addition, staff are competent in issues related to caring for transgender patients.
