ACCESS provides a continuum of care model that connects patients to health care resources both within and beyond the walls of our 35 federally-qualified health centers (FQHCs). ACCESS' services are designed to address the health of our underserved communities in such areas as preventive care, chronic disease management, and support services. To address patients' comprehensive health needs, ACCESS physicians, nurse practitioners, midwives and other providers are teamed with outreach staff, case managers, social workers and substance abuse counselors to advance a continuum of care.

Details: Beloved Community Family Wellness Center (BCFWC) is a not-for-profit Federally Qualified Health Center committed to providing comprehensive, accessible, timely, and affordable primary health care, preventive education, and social service programs available to the Greater Englewood and surrounding communities. Details: CHRISTIAN COMMUNITY HEALTH CENTER is a community-based 501 (c) 3 not-for-profit organization founded in 1991 to address the lack of health care in the Roseland community on Chicago's southside. The agency was formed with a MISSION to provide high quality primary healthcare and related services to the community regardless of ability to pay, in a manner which demonstrates in word and deed the love of Jesus Christ. Since receiving Federally Qualified Health Center (FQHC) status in 2001, CCHC has propelled an expansion of programs and services. While primary medical care is our anchor program, CCHC has become equally renowned for our presence as a multi-pronged social service agency. By incorporating primary medical treatment into a comprehensive slate of supportive programs, CCHC strives to meet the holistic needs of our community residents. CCHC currently operates seven sites throughout the Chicago land area, including four community health centers, as well as a mobile health van providing a wide range of primary medical and social services.

Details: In 2004, residents of the neighborhoods of Pilsen and Little Village on Chicago’s southwest side joined administrators from nearby Saint Anthony Hospital to address the critical lack of accessible health services in the area. They opened a community health center on South California Avenue called Esperanza, the Spanish word for “hope.” Today, Esperanza provides bilingual, high quality primary care, behavioral health and wellness services to the community, regardless of immigration status, insurance status, or ability to pay. Esperanza’s three sites in Little Village (South Lawndale) and Marquette Park (Chicago Lawn) deliver care to over 20,000 patients each year from communities across the Southwest side.
