About this clinic

Boricua Community Health Center is the newest site in Urban Health Plan’s growing network of community health centers, having opened on April 6, 2015 in the Morrisania section of the Bronx. In a community with high rates of diabetes, hypertension and heart disease, Boricua offers primary care services for all ages, including podiatry, ophthalmology, psychology, nutrition and health education. In addition, Boricua offers an array of social services that help address economic development issues, including poverty and unemployment, with both on-site assistance and referral services. It also has an on-site facilitated enroller to help uninsured patients determine their eligibility for different health insurance options.
