Clinic Details:Our mission: Provide quality healthcare throughout our communities. In 1978 Wirt County Health Services Association (d.b.a. Coplin Health Systems) was established as a non-profit organization. Since then, Coplin Health Systems has expanded into seven different facilities located in Wirt County, Jackson County, Wood County in West Virginia and Meigs County in Ohio, including three school-based clinics. Coplin Health Systems specializes in primary care services. Our healthcare providers offer a full range of services for the entire family. We provide access to preventative and quality primary care services, as well as acute care services, regardless of one’s ability to pay. We are advocates for children and families, alongside the communities and strive to give them the best possible care.

Since 1979, we've dedicated our services to providing the best healthcare needs to the children and adults in Princeton and surrounding towns. This center is partially funded by US Public Health Services and the WV Bureau for Public Health and Black Lung Grant monies. Bluestone Health Association has Mal-Practice coverage under Federal Tort Claim Act (FTCA) through Federal Government.
Details: Mountain Laurel Integrated Health serves the Citizens of Logan and Mingo Counties. We are a nonprofit comprehensive community provider of behavioral health services. Our staff of trained professionals provides an array of services for children, adolescents, and adults. We have locations in Logan and Mingo Counties. Details: Shenandoah Community Health (SCH) is a Federally Qualified Community Health Center and Primary Care Medical Home that offers access to primary care, behavioral health, women’s health, and oral healthcare right here in the Shenandoah Valley of West Virginia and Virginia. Since 1976, our dedicated clinicians, nurses, and support staff have worked to ensure that our patients receive the highest quality, most comprehensive healthcare possible. SCH’s wide range of coordinated healthcare services includes routine care as well as chronic disease treatment and management; mental health and substance use treatment; specialized care for women including prenatal care and delivery, gynecology and surgery; and comprehensive dental care for children and adults. As a medical home, SCH also offers WIC Nutrition Programs; HIV case management; weight and wellness program; on-site pharmacy; lab and x-ray services; nursing outreach to migrant and seasonal farmworkers and the homeless; medication assistance; assistance with health insurance enrollment; social work services; and family support resources. Health education classes, a breastfeeding support group and diabetes management program are offered free of charge and are open to the public. Shenandoah Community Health serves all patients - adults and children - regardless of ability to pay. We also provide interpretation services for patients whose primary language is not English.
Details: Community Care of West Milford offers an array of medical services including comprehensive primary care and chronic disease management for the entire family. This location also has several healthcare programs to assist you with your medical needs, as well as convenient mail order pharmacy services. Community Care of West Milford has received a Level 3 accreditation - the highest possible - from the National Committee for Quality Assurance (NCQA) for providing exceptional service and high quality care. For acute care for non-life-threatening illnesses and injuries after hours and weekends, please visit our CareXpress location in Clarksburg. CareXpress is open seven (7) days a week, because we know that illnesses and injuries don’t occur at convenient times. That is why we don’t make appointments. Just walk in when you need us, and you will be seen. Quickly. The CCWV chronic disease management model provides for a complete range of on-site services for comprehensive patient care. The mix of CCWV’s medical providers and nurses provide assessment and education so patients (and their families) better understand their illnesses and what they can do to help themselves, including any referrals for additional care that may be necessary. CCWV’s Care Managers and the patient’s medical provider work closely together to give the patient the best care management possible. As part of this expanded care management assessment, the health history of all patients is reviewed to assure that each has been screened for chronic conditions including high blood pressure, high cholesterol, depression, weight management, physical activity, and asthma. Other risk factors, such as pregnancy, co-morbid conditions, or family history of disease, are also assessed. CCWV offers family planning services and participates in BCCSP. For children, immunization are available through the VFC program and private insurances. Our state-of-the-art Electronic Health Record (EHR) and a team of healthcare professionals are focused on chronic disease management and quality care outcomes throughout their system of Community Health Centers. Our EHR enables the medical providers to quickly access and manage clinical information such as medical history, progress notes, laboratory results, diagnostic imaging, medication orders, and clinical consultations. Most importantly, the EHR provides key reminders and tracking measures that allow the providers to monitor the patients’ health in order to detect small problems as they arise and provide preventive services on the front end.
