Clinic Details:Are you experiencing homelessness? If so, you are not alone. The Community Health Center of the Black Hills Health Care for the Homeless (HCH) can provide the care you need. HCH serves people who lack stable housing. This includes people living in shelters, transitional housing, on the streets, or “doubled up” with friends, neighbors, or relatives.
Services:Primary Care; Mental Health; Oral Health (By Referral to Oral Health Center); Substance Abuse Detoxification Referral; Family Planning (By Referral to Health Center); Lab (By Referral to Health Center); X-ray (By Referral to Health Center), etc.
Details: Located in a close knit community, the Howard Community Health Center provides a full range of family medicine. Our providers make on site visits to the Howard Good Samaritan Center and Whispering Winds Assisted Living Center to provide health care to their residents. Community Counseling Services from Madison, SD has staff on site weekly to provide counseling services. The community of Howard is also fortunate to have a pharmacy, physical therapy services and a volunteer ambulance service. Howard Community Health Center accepts all insurance and offer reduced prices on office visits and all other services provided in the clinic to all individuals and families who qualify through the Sliding Fee Program.
Details: Prairie Winds Dental is a fully equipped dental suite consisting of five dental operatories. We offer digital x rays for the three intraoral units and also are equipped with a panoramic/cephlametric x ray unit. The clinic provides a full range of family oral health services including preventative care, restorative and cosmetic dentistry, endodontics, and oral surgery. The dental clinic is staffed with two dentists and two hygienists.
Details: Alcester Dental Clinic began serving the Alcester community in 2015 and became a Horizon Health Care location in 2017, following a merger with allPOINTS Health Services. Alcester Dental Clinic is only open on Thursdays.

Details: Servant Hearts Clinic is a Christ-centered FREE health care clinic dedicated to providing health and hope to all we serve. As an urgent care provider, Servant Hearts Clinic plays a role in helping patients who have limited access to health care. This access may be limited by income, language, legal status, insurance access, and/or many other variables.
Details: Rural Health Care Inc., in collaboration with Avera Health, operates the Kennebec Clinic in Kennebec, SD. This collaboration of two South Dakota based health care delivery systems strives to meet the needs of the region; including serving the underinsured and underserved.
Details: Faith Community Health Center began operations in 1998 and joined Horizon Health Care Inc.’s clinics in 2016 through a merge with Prairie Community Health. The clinic is equipped with three exam rooms, one treatment room, laboratory facilities and digital x-ray technology. The Faith Community Clinic is staffed with two Physician’s Assistants, Alanna Chapman, PA-C, and Valarie Brown, PA-C.

Dakota Family Medical Center offers health care to all ages, from infant care to geriatrics. DFMC is an All Women Count provider, State Immunization program participant, and offers a sliding fee discount program. The health center offers monthly “free immunization” clinics, as well as reduced rates for health maintenance screenings at least semi-annually.
