About this clinic

At Creek Valley Health Clinic, our mission is to provide exceptional healthcare for every patient, every time. Our clinic has a goal to make our community strong, empowered, and healthy through delivering healthcare services, health education, and vital resources to all residents, regardless of ability to pay. Creek Valley is a 501(c)(3) nonprofit community health center managed by a Board of Directors comprised of individuals who live and work in ‘Creek Valley’ (Apple Valley, Hildale, Colorado City, Centennial Park, and Cane Beds). In order to best serve the community, Creek Valley operates with a Program 330 grant and is included in the national Health Center Program, governed by the Health Resources and Services Administration. As a community health center, Creek Valley increases access to primary care services by reducing healthcare barriers such as cost, lack of insurance, distance to receive services, and potential language barriers for patients. Creek Valley offers many services including annual checkups, well-child exams, sports and kindergarten physicals, vaccinations, family planning, counseling and therapy, psychiatry services, immunizations, well-woman exams, cancer screenings, prenatal and postnatal care, and MANY other preventive medical services. In addition to direct primary care, Creek Valley has established agreements and relationships that improve pharmacy and medication access, laboratory services, imaging (X-ray, CT, MRI, Mammograms, etc.), health education, transportation assistance, insurance eligibility assistance, and many other supporting services. Creek Valley Health Clinic accepts same day and walk-in appointments for all of your urgent medical needs.
