Clinic Details:In 1998, AAPHC begin providing services to the residents of Terrell County at DMC. DMC provides health care for the entire family with providers specializing in Family Medicine, Internal Medicine, and Pediatrics. DMC also provides patients with on-site access to Podiatric and Women’s Health services on a scheduled monthly basis with adjunct physicians. DMC is also open on Saturday mornings to provide additional access to care for their patients.

Details: Fayette C.A.R.E. Clinic opened its doors on May 1, 2006 in Fayetteville, GA. Providing only medical care, and open only one evening each week, we began serving the uninsured and working poor in the community. Since that time our organization has grown in significant ways. We've expanded our services, increased our hours of operation, hired more staff, developed relationships with more volunteers and community partners. As a result, we're able to help much more of the underserved population in Fayette County. Our staff includes skilled, licensed, health care providers who volunteer their time and talents to serve our patients. Additional volunteers from the community help at the clinic with a wide range of tasks.
Details: We exist to fill a gap in health care services, primarily for individuals and families at or below 200% of the federal poverty level. Unlike a free clinic, our charges for this population are based on a sliding fee scale that takes into consideration the number of persons in the family and their total income. We provide primary health care to anyone and accept most insurance plans, including Medicaid and Medicare. Patients are not turned away, regardless of their ability to pay! Our staff treats the whole person, not just the presenting symptom or problem. As a Diversity patient, you will be an active member of your health care team. In addition to pediatric, adult and family medical care, Primary Healthcare Centers provides X-ray services and general dentistry services for children and adults at this location. Medical services are available on Saturdays.

Details: The Georgia Ellenton Farmworker Health Clinic is located in Ellenton city, Colquitt County. The clinic is federally funded through the Georgia Department of Community Health (DCH) and the Boards of Health in counties being contracted by DCH to provide primary health care to farmworkers and their families. To qualify for the program a total family annual income of 51% must come from agriculture. The client must provide proof of income and will be charged on a sliding scale basis.
