Clinic Details:The health services that became the Anthony L. Jordan Health Center, began more than 100 years ago, in 1904, and was one of the first five Federally Qualified Health Center (FQHC) established in the nation. Located in neighborhoods where the most pressing need exists, Jordan’s roots are steeped in service to underserved and uninsured residents, meeting their need for comprehensive health services. Anthony L. Jordan Health Center began in northeast Rochester and has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants of the Rochester and in Canandaigua, N.Y.

Details: Mosaic Health serves patients from Monroe County and the surrounding area through partnerships with community organizations, Rochester General Health System, and Unity Health System. There are currently 19 Rochester-based locations in the Mosaic Health Network, including 5 School-Based Health Centers.
Details: Healthcare for the Homeless Program (HCHP) provides quality comprehensive primary medical care to the homeless persons in the city of Rochester and the greater Rochester area. Seeking to improve access to comprehensive medical care for homeless individuals, while simultaneously affording each individual access to services needed to obtain gainful employment, permanent housing and other resources that may be beneficial to their individual situation, without regard to the individual’s ability to pay.

Details: Our School-Based Health Center provides medical and behavioral health services within a safe, comfortable school environment, so that high-quality care is within easy access to students regardless of socio-economic status. Services in this program are provided at no direct cost to the student or family. Costs are covered by your health insurance, the New York State Health Department, federal grants and Rochester Regional Health grants. The School-Based Health Center is staffed by caring and dedicated Licensed Social Workers/ Mental Health Therapists, Psychiatrists, Nurse Practitioners, Pediatricians and Medical Assistants, Outreach Workers and Office Assistants. These professionals are committed to providing care and support for students in need, and to working closely with the family’s regular doctor in the community to ensure that each student receives consistent care.
