Clinic Details:For more than 25 years, ACCESS has been on the frontlines of community-based health care. ACCESS provides a continuum of care model that connects patients to health care resources both within and beyond the walls of our 35 federally-qualified health centers (FQHCs). ACCESS' services are designed to address the health of our underserved communities in such areas as preventive care, chronic disease management, and support services. To address patients' comprehensive health needs, ACCESS physicians, nurse practitioners, midwives and other providers are teamed with outreach staff, case managers, social workers and substance abuse counselors to advance a continuum of care.

Details: Near North Health Service Corporation (NNHSC), a 501(c)(3) non-profit Federally Qualified Health Center (FQHC), is one of the largest providers of community-based primary health care in Chicago. We provide medical services, social services, and nutrition education to medically indigent and uninsured individuals and families. Our philosophy reflects an approach that anticipates community needs and plans for expansions that are developed in accordance to these needs. Details: In 2010, PCC Austin Family Health Center was relocated to a brand new two-story, 18,300 square-foot building. PCC Austin Family Health Center is one of the most environmentally friendly community health centers in the country. The facility has achieved Leadership in Energy and Environmental Design (LEED) Gold certification from the U.S. Green Building Council. Across the street from the health center is the PCC Austin Farm.

Details: Behavioral Health Services of Circle Family HealthCare Network provides services to adults and children who are diagnosed with serious mental illness, emotional disturbances, and/or have significant loss of normal functioning. Outpatient services are provided from our health center at 5002 W. Madison St. Residential care is provided for up to eight (8) CILA (Community Integrated Living Arrangement) adult clients through an off-site location as part of a special program.
Details: PCC Community Wellness Center at West Suburban is now located on the 7th floor of West Suburban Medical Center's Professional Office Building. Patients can easily come to appointments for primary care and behavioral health care.
Details: PCC Walk-In Wellness Center at West Suburban Medical Center offers urgent care for illnesses and injuries that are not true emergencies. Patients can be seen without a scheduled appointment for a variety of health conditions. In addition, any patient that does not have a primary care provider can follow up with a primary care provider at one of our health centers and make PCC their medical home.This is also the location of our Chemical Dependency Clinic, Ultrasound Clinic, and Sports Medicine Clinic.

Details: PCC Salud Family Health Center moved into a newly renovated facility on April 8, 2013. The facility includes a large patient waiting and registration area, newly renovated exam rooms, offices and restrooms, as well as new windows, flooring, lighting, plumbing, roof, heating and cooling system, and building entrances with signage. As a result of the renovations, PCC Salud Family Health Center meets the requirements for Leadership in Energy and Environmental Design (LEED) silver certification from the U.S. Green Building Council.
